Please complete the form below so that we can customize your child’s birthday celebration. We will be in touch to confirm after this form is completed. Please complete as early as possible (up to six months in advance)! Please note we are a very small organization, and usually reach capacity each month- so get your birthdays in early. We also require a minimum of at least three weeks’ notice! Children must be active in the foster care system (not adopted). We are fully booked through January 2025, accepting submissions for February and later only.
